New Patient Paperwork

Thank you for choosing Premier! We want to make your first visit as seamless as possible. As a new patient, you will need to fill out a Medical History, a Patient Policies and Procedures form, a Prescription list and a pain scale relating to your specific condition. The majority of your paperwork is linked below, if you’d like to print and complete the forms ahead of time. If you prefer to complete them at your time of appointment, please make sure to arrive 15-20 minutes early. See you soon!
